BEFORE REGISTERING FOR A LODESTAR INSTITUTE COURSE USING THE FORM ABOVE, YOU MUST READ AND UNDERSTAND THE TERMS AND CONDITIONS.
TERMS AND CONDITIONS
Payment: All payments must be received by LodeStar Institute before the beginning of the first session. If payment is not received according to the terms specified, LodeStar Institute reserves the right to release your reservation.
You may register on-line, by phone, mail, using this registration form.
With your registration, please include a check, money order, or company-issued purchase order number. Please note that company purchase orders must be submitted prior to the time of registration. If you wish to pay by credit card, please call our office at 1.919.772.1132, a transaction fee may apply.
Discounts: For registrations of multiple participants from the same organization, for the same course, LodeStar Institute offers the following discounts:
2 participants = 10% discount,
3-4 participants = 20% discount,
5-6 participants = 30% discount,
7 or more participants = 40% discount.
Please call our office for groups of 10 or more and on-site pricing. Only one discount per person or group.
All orders must be paid in U.S. dollars and drawn on a U.S. financial institution.
If your company has a purchase order for the training, LodeStar Institute will invoice your company for payment. Terms of the payment are net 30 days with a 2.0% late penalty per month after 30 days from the date of invoice. If collection or legal action is necessary, your organization is liable for any legal or other collection costs.
Cancellation: Substitutions are allowed up until one day prior to the start of the course. Cancellations are allowed until the end of the business day four weeks prior to the start of the first day of the course with a 25% cancellation fee. A 75% cancellation fee or $750, whichever is greater but not to exceed the cost of the course, will be assessed if the cancellation is made within four weeks but prior to two weeks before the first day of the course. Registrants who fail to attend or cancel within two weeks of the first day of the course are liable for the entire course fee. Please be aware that you are responsible to attend each session for the course that you have registered for. In the case of an emergency, where you may not be able to attend a session, please contact LodeStar Institute prior to the session. LodeStar Institute will attempt to reschedule you, but the timing for an alternate class is not guaranteed.
LodeStar Institute reserves the right to cancel or reschedule any course. Please be advised that LodeStar Institute is not responsible for any airfare or other travel penalties or charges that you may incur in the event that a course must be cancelled or rescheduled. If a course is rescheduled, the cancellation policies remain unchanged based on the original start date.
Computer/Software Requirements: All LodeStar Institute training courses, except our Executive Overviews, require laptop computers. Prior to the beginning of your session, you should secure a laptop comparable to Pentium III or greater with Microsoft Windows 2000 or greater with all service packs installed. Microsoft Office 2000 or greater with all service packs installed, Minitab Release 14 or greater, access to e-mail/appropriate software. It is your responsibility to check the software requirements for the course and purchase the necessary software.
